A legal disclaimer

All services, coaching services and communication, email or otherwise, delivered by AwakenUp LLC, DBA AwakenUp, as well as information set forth on this website (www.awakenup.net) are meant to help you identify the areas in your life and your thinking that may be standing in your way. However, coaching is not professional mental health care or medical care. If you feel psychologically stressed to the point that it is interfering with your ability to function, please seek help from a mental health provider. 

In that spirit, by purchasing services from AwakenUp LLC you confirm that you have read and agree to each statement: 

I understand that the services I will be receiving are not a substitute for mental health care or medical care, and are not intended to diagnose, treat, or cure any mental or medical conditions. I also understand that AwakenUp LLC is not acting as a mental health counselor or a medical professional.

I understand that coaching is presently an unregulated industry and my coach is not licensed by the state of Oklahoma or any state. I understand for legal purposes the services provided will be considered provided in the state of Oklahoma. 

I understand and agree that I am fully responsible for my well-being during any sessions or services and subsequently including my choices and decisions. I understand that coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment and will not use it in place of any form of therapy. 

I understand all comments and ideas offered by AwakenUp LLC are solely for the purpose of aiding me. I give my informed consent and hereby give such consent to assist me in achieving goals.  I understand AwakenUp LLC is not promising results.

I understand AwakenUp LLC will protect my information as confidential unless I state otherwise. If I report child or elder abuse, neglect, or threaten to harm myself or others I understand necessary steps will be taken and my confidentiality agreement will be limited in this capacity. Furthermore, if ordered by court to provide information or testify it will be done so by the extent the law requires.

I understand that use of   technology is not always secure, and I accept the risks of confidentiality in the use of email, text, phone, zoom, skype and other technology.

I hereby release, waive, acquit and forever discharge AwakenUp LLC, my coach, and any agents, successors, assigns, representatives, executors, heirs and employees from every claim, suit, action, demand, or right for compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by AwakenUp LLC as a result of advice given or otherwise resulting from the relationship contemplated by this agreement.  I further declare and represent no promise, inducement, or agreement not expressed in this agreement has been made to me to sign this agreement. This agreement should bind my heirs, executors, successors, assigns, and agents.
